Citizenship Status Documentation Needed for Marketplace Insurance Coverage

You would need to upload these documents because the federal government has requested additional documentation to verify your citizenship to complete your application for Marketplace insurance coverage. This is a common request, and as long as the required documents are submitted by the specified deadline, your health insurance coverage should not be impacted.

Once your documents are uploaded, our team will submit the documentation to the Marketplace for you.

We appreciate your prompt attention to this matter. Delays in submitting the requested documentation could result in interruptions or termination of your insurance coverage.

Primary Documents (submit one of the following):

U.S. Passport
Certificate of Naturalization (Form N-550 or N-570)
Certificate of Citizenship (Form N-560 or N-561)
State-issued Enhanced Driver’s License (available in Michigan, New York, Vermont, and Washington)
Documentation issued by a federally recognized Indian tribe that includes your name, the name of the tribe, and proof of membership, enrollment, or affiliation. Acceptable tribal documents include:
- Tribal enrollment card
- Certificate of Degree of Indian Blood
- Tribal census document
- Documents on official tribal letterhead signed by a tribal official

If none of the above documents are available, you may submit two documents, one from each of the categories below:

Category A: Proof of Citizenship (submit one):

U.S. Birth Certificate
Consular Report of Birth Abroad
Certification of Report of Birth
Certification of Birth Abroad (Form FS-545)
U.S. Citizen Identification Card (Form I-197 or prior version I-179)
Northern Mariana Card (Form I-873)
Final adoption decree showing the individual's name and U.S. place of birth
U.S. Civil Service Employment Record (dated prior to June 1, 1972)
Military record indicating U.S. place of birth
U.S. medical record from a clinic, hospital, physician, midwife, or institution showing U.S. place of birth
U.S. life, health, or other insurance record showing U.S. place of birth
Religious record showing U.S. place of birth, recorded in the United States
School record showing the student’s name and U.S. place of birth
Federal or state census record showing U.S. citizenship or U.S. place of birth
Documentation of a foreign-born adopted child who received automatic U.S. citizenship (IR3 or IH3 classification)

Category B: Proof of Identity (submit one — must include a photograph or identifying information such as name, age, race, height, weight, eye color, or address):

State- or territory-issued driver’s license or government-issued identification card
School identification card
U.S. military card, draft record, or military dependent’s identification card
U.S. Coast Guard Merchant Mariner card
Voter registration card
Medical, hospital, clinic, or school records (including preschool or daycare records for children under 19)
Two documents containing consistent identifying information, such as:
Employer identification cards
High school or college diplomas
Marriage certificates or divorce decrees
Property deeds or titles
