Citizenship Status Documentation Needed for Marketplace Insurance Coverage
You would need to upload these documents because the federal government has requested additional documentation to verify your citizenship to complete your application for Marketplace insurance coverage. This is a common request, and as long as the required documents are submitted by the specified deadline, your health insurance coverage should not be impacted.
Once your documents are uploaded, our team will submit the documentation to the Marketplace for you.

Primary Documents (submit one of the following):

U.S. Passport

Certificate of Naturalization (Form N-550 or N-570)

Certificate of Citizenship (Form N-560 or N-561)

State-issued Enhanced Driver’s License (available in Michigan, New York, Vermont, and Washington)

Documentation issued by a federally recognized Indian tribe that includes your name, the name of the tribe, and proof of membership, enrollment, or affiliation. Acceptable tribal documents include:

Certificate of Degree of Indian Blood

Documents on official tribal letterhead signed by a tribal official
